	“A successful palliative and end-of-life program heavily relies on identification, stratification and engagement of members/patients at least a year, and preferably two years, in advance. Predictive analytics could aid in such efforts, but for such a long range prediction with appropriate sensitivity/specificity, underlying data needs to go beyond traditional administra-tive healthcare claims data sources and needs to include electronic medical record data, including clinical and home assessments. Social determinants of health/lifestyle data at the person and household level would be a good supplemental data source. Some of the clinical data (e.g. albumin level) could be highly predictive of end of life.
