  “A successful palliative and end-of-life program heavily relies on identification, stratification and engagement of members/patients at least a year, and preferably two years, in advance. Predictive analytics could aid in such efforts, but for such a long range prediction with appropriate sensitivity/specificity, underlying data needs to go beyond traditional administra-tive healthcare claims data sources and needs to include electronic medical record data, including clinical and home assessments. Social determinants of health/lifestyle data at the person and household level would be a good supplemental data source. Some of the clinical data (e.g. albumin level) could be highly predictive of end of life. Also, leveraging advanced predictive modeling techniques such as machine learning could be very effective.”
