Clinical model predicts which patients require prolonged mechanical ventilation

In a recent study, 31.3% of adult patients in an ICU who required mechanical ventilation (MV) at the time of admission, continued to require it for 14 or more days, a condition known as Prolonged MV or PMV. Specificity for predicting PMV was 100% in patients who had 4 or more of the following variables, which were the only ones independently associated with PMV: requiring intubation after admission to the ICU, heart rate greater than 110, blood urea nitrogen more than 25 mg/dL, serum pH less than 7.25, serum creatinine more than 2.0 mg/dL, and a HCO3 less than 20 mEq/L were the only variables.
