"Over the past several
years, two predictive modeling initiatives that have had significant
impact on healthcare processes and outcomes are (a) member
identification, stratification and triaging for care management and (b)
risk assessment and risk adjustment in support of payment innovation
efforts for the population health management and shared
savings/incentives/revenue.... Proactive identification of high-risk
members and addressing their care needs in a timely fashion has also led
to cost-of-care savings. Payment innovation efforts and delegation of
risk management from payers to providers have created partnerships
between payers and providers around sharing payer informatics and best
practices for population health management. Attributed population
underlying illness burden and its impact on savings/incentives/revenue
has been addressed by payers through risk adjustment. Such use of
predictive modeling has led to better adoption of payment innovations
