  “Essentially MACRA ends the special growth rate (non-)adjustment that was imposed in the mid-1990s and replaces it with an ACO-type solution. My experience of predictive modeling in ACOs was that it was difficult to get the physicians to understand what predictive modeling could do for them, or to understand what ‘high-risk’ meant. At the same time, physicians were very aware of the financial rewards that came from quality and meaningful use, which appeared larger and a lot more real than ACO gains, with the result that it was difficult to focus physicians on predictive modeling and their attendant programs. The purpose of the whole initiative is to further the penetration of ‘value-based purchasing’ into medicine. Predictive modeling’s cousin, risk adjustment, has a role to play in adjusting quality measures for underlying relative patient risk. But I am still not convinced that the measures that are reported actually measure value. Time will tell. Certainly judging by letters to the Wall Street Journal’s editorial page physicians are aware of the additional administrative burden.”
