Reducing Hospital Readmissions for Congestive Heart Failure

"The great challenge will be learning what leads to avoidable readmissions. For example, we will need to understand what led to a patient's admission in the first place in order to assure that we have addressed the circumstances that precipitated the admission. If there are
medical, social, or environmental issues that compromise the patient's health, we need a plan to mitigate those issues by the time the patient is discharged. Otherwise, there will be no reason to believe we can avoid a readmission.” Martin S. Kohn, MD, MS, FACEP, CPE, FACPE, Chief Medical Scientist, Care Delivery Systems, IBM Research

"Analytics are big. Overall, roughly three-fourths (74.4%) of respondents have analytics that help them identify patients at high risk of readmission, while another 17.9% don’t currently have the capability now but are working on it. Only 7.7% said no."
