FMT Innovative Projects Webinar: Care Coordination

This webinar focuses on care coordination and quality improvement. Speakers include: Pat Schou (IL); Jill Oesterle (MI); Bill Mize and Richard Taylor (TN); and Marcy Cameron (CO). Schou discusses the Illinois Rural Community Care Organization's efforts to improve communication between health care facilities when patients are being transferred. Oesterle talks about the newly formed Michigan Rural Health Clinic Quality Network. Mize and Taylor discuss efforts to help rural practices in Tennessee prepare for value-based purchasing. Finally, Cameron talks about Colorado's iCare program, which looks to improve communication, reduce readmissions, and improve clinical processes.
