"Examining social determinants of health does not need to be overly
complex. The hospital I previously worked for built a navigation program
aimed at very simple activities, such as going through patients’
cabinets to see their food choices. It was common to discover that
patients with breathing problems didn’t have air conditioners, or those
with heart problems often didn’t understand the need to monitor their
daily sodium intake. These are simple things to figure out — if you know
where to look. And the impact can be huge.” - Michael Ipekdjian, MBA-HM, BSN, RN, Director of Customer Success, CarePort Health. |

"The readmission rate
for patients participating in the HELM [Health Education and Lifestyle
Management] program is 50% lower than the readmission rate of patients
not using telehealth at VNA Philadelphia. Patients placed on the HELM
program have also reported high satisfaction with the telehealth
software. On a five-question patient satisfaction survey, 81.25% of
patients reported that they strongly agreed or agreed that they were
satisfied with the telehealth program." Excerpted from: Readmissions News, Volume 8, Number 4, April 2019, Industry News: "VNA Philadelphia Lowers Hospital Readmissions and Expands Telehealth Program with HRS." |
