  "The readmission rate for patients participating in the HELM [Health Education and Lifestyle Management] program is 50% lower than the readmission rate of patients not using telehealth at VNA Philadelphia. Patients placed on the HELM program have also reported high satisfaction with the telehealth software. On a five-question patient satisfaction survey, 81.25% of patients reported that they strongly agreed or agreed that they were satisfied with the telehealth program."
