Care Transitions: helping Joann stay out of the hospital  

Joann was in the hospital for 10 days with heart problems. Soon after Joann was discharged, health coach Alisa Philipps met with her in her home. All too often, seniors bounce in and out of the hospital. Readmissions can be prevented with the help of coaches who help patients use their medications properly; recognize and act on symptom "red flags"; follow up with their doctor; and connect to community resources such as in-home care.
