"COPD is responsible for nearly 700,000 hospitalizations annually, and
hospitalizations account for a large proportion of the annual direct
medical costs of COPD. About 20% of patients hospitalized with COPD
exacerbations are rehospitalized within 30 days of discharge, and these
rehospitalizations are costly.” -Jane H. Huntington, Associate Professor, Family Medicine, University of Washington School of Medicine. |

"Over a three-month
period, following an integration phase, 40 patients were enrolled in the
heart failure protocol. Of these, only three (8%) were readmitted to the
hospital with a cardiac-related diagnosis. Contrast these results with a
similar three-month period prior to protocol initiation when 36 patients
with a diagnosis of congestive heart failure were admitted. Of those, 11
(31%) were readmitted with a cardiac-related diagnosis." Excerpted from: Readmissions News, Volume 6, Number 6, June 2017, "New Strategy for Reducing Readmissions: Get the Family Involved." |
