"Effective medication reconciliation and patient education strategies are vital ways for hospitals to reduce readmission rates. Poor health literacy among American adults is responsible for far too many unnecessary rehospitalizations in the United States. This is particularly true as it relates to medication, where adverse drug events can lead to readmissions and worse. Implementing strategies that work to effectively educate patients as well as deal with medication reconciliation are important means through which a health care organization can reduce the potential for significant monetary penalties associated with readmission rates. Success in these two areas will result in higher quality and improved patient safety.
