Analysis of Skilled Nursing Facility Transfers Using a Quality Improvement Tool

Report on the root cause analysis data of 3,668 nursing home resident hospitalizations occurring over nearly three years. This is a mixed method descriptive study of the root cause analysis INTERACT 3.0 tool entered into a Qualtrics electronic survey platform. The Missouri Quality Improve-ment (MOQI) initiative partners with 16 St. Louis area nursing homes to reduce avoidable hospital admissions/ readmissions and cost, and improve health outcomes.
