"The highest risk group today is senior orphans for 30 day readmissions; the next highest group is adults with disabilities without adequate family support. The most frequent reason is often medication noncompliance or side effects of new medicines, as well as infections. Older adults with no one to help them at home after an illness can often get confused, fall, become dehydrated or malnourished with no one around to notice.”
- Teri Dreher, RN, CCRN, iRNPA, Owner and CEO, NShore Patient Advocates, Chicago.

The Sanger Heart & Vascular Heart Care Navigation Team study examined a program designed to help heart attack patients with the transition from hospital to outpatient care. "The 30-day readmission rate before the program started was 6.3 percent and fell to 3.7 percent the year after the program began. There was a reduction in the 30-day death rate (5.75 percent before vs. 4.57 percent after program implementation) and an increase in patients' follow-up appointments made prior to discharge (78 percent vs. 96 percent)."
