Beacon Community Research Study: Reducing Hospital Readmissions via Remote Patient Management   

Central Indiana hospitals are addressing readmission rates by launching a program to identify patients discharged with a diagnosis of CHF or COPD in a controlled trial of tele-home monitoring for 30 days. The St. Vincent Nurse Contact Center monitors these patients daily to review their results, help address concerns and educate patients and family members.
