"Convincing hospital leadership of the importance of readmission
reduction is no longer a hard sell as the changing financial incentives
for hospitals with high rates of readmission has certainly motivated
them to focus a great deal of attention on this issue. Across the
country there is now a long list "early adopter" hospitals that have
demonstrated remarkable reductions in readmission rates. All this effort
is forcing hospitals and communities to work together as partners, a
necessary ingredient for successful ACOs. However, safe readmission
reduction can only happen if hospitals have well developed
community-based partners, particularly primary care partners, willing
and able to care for patients in the community. More effort to insuring
