Factoid
  A retrospective study of 7,507 adult patients discharged from the hospital between January and December 2018, found 5.2% of patients had an unplanned readmission to the same hospital within 7 days of discharge. 5.6% of the 7-day readmissions had one or more diagnostic errors. The most common diagnostic errors included failure to order a needed test or test (52.4%), erroneous clinician interpretation of a test or tests (47.6%), and failure to consider the correct diagnosis (38.1%).
