that the number of hospitals facing financial penalties would be high
as, historically, the medical community largely approaches health care
in a reactive way – that is, it reacts to patient needs rather than
anticipating them. Until the patient-centered medical home model is more
widely accepted, enhanced, and implemented, a system will not be in
place that consistently and effectively anticipates patient needs
before, during, and after a hospital admission. Therefore, the number of
readmissions will likely continue to remain high in the short term.”
