  "Early follow-up with a patient's primary care provider (PCP) and/or medical specialist after hospital discharge has been shown to be effective in preventing 30-day readmissions in patients with chronic and acute medical conditions such as heart failure and pneumonia...Patients who went to see their PCP within the first couple weeks following discharge for a high-risk surgery such as open TAA repair were significantly less likely to require readmission."
