"In well coordinated and connected physician organizations, the primary care provider with the support of other health care staff can act as a unifying hub or resource that identifies the ongoing medical, social, perhaps behavioral and spiritual needs to lead the coordination of care. This would require outreach, cross-continuum communication, monitoring and customized interventions that are supported by high-level information technology. A PCP with this type of support infrastructure and community relationships could be highly effective at reducing all types of avoidable admissions and readmissions." -Bruce Spurlock, Executive Director, Clinical Acceleration, Beacon, The Bay Area Patient Safety Collaborative; President and Chief Executive Officer, Convergence Health Consulting, Inc., Roseville, CA |

"Early follow-up with a patient's primary care provider (PCP) and/or medical specialist after hospital discharge has been shown to be effective in preventing 30-day readmissions in patients with chronic and acute medical conditions such as heart failure and pneumonia...Patients who went to see their PCP within the first couple weeks following discharge for a high-risk surgery such as open TAA repair were significantly less likely to require readmission."
