  “Patients with CKD [chronic kidney disease], and ESRD [end-stage renal disease] experienced rehospitalization at rates of 21.4 percent and 35.2 percent, as compared to only 15.4 percent
for older Medicare beneficiaries without a diagnosis of kidney disease. ESRD patients, on average, are admitted twice a year, and more than one-third face rehospitalization within 30 days following discharge."
