Factoid
  One health system using EmmiTransition’s Voice User Interface technology to follow up on for chronic obstructive pulmonary disease (COPD) patients found that patients who interacted with Emmi experienced a 15-day delay in readmission than patients who did not receive the phone calls. Their readmission length of stay was reduced by nearly 70%, as well.

Dr. Sean Pinney from Mount Sinai discusses the value of following up with patients in the first 7 days after their hospital discharge. Utilizing ReDS to assess fluid at this stage is critically valuable to make adjustments to medication in patients with relapsing heart failure. These adjustments help to reduce the likelihood of a readmission.

Bobby Stephenson: 4 Ways to Reduce Hospital Readmissions for Sepsis Survivors

1. Improved Post-Discharge Processes: most patients require post-acute care, such as home health services or admission to a skilled nursing facility.
2. An All-Hands Approach to Post-Sepsis Care: a post-discharge protocol that combines follow-up appointments with both a physician and a nurse.
3. Better Education on Sepsis for Patients and Caregivers: patients are unusually susceptible to recurring or new infections.
4. Rehabilitation Referrals: referral to rehabilitation within 90 days of discharge from hospital has been correlated with lower 10-year mortality risk.
