  "One method to reduce hospital readmission, through patient engagement, is to identify patients at risk for readmission before they are discharged from the hospital. This is done through early engagement of patients and their families prior to discharge. This process includes identification of high risk patients and classification of risk of readmission into categories: very high, high, medium and low. It might also include specific follow-up for specific diagnoses which have a high likelihood for readmission. Patient engagement is important to improve the discharge process. Early identification of a patients care needs is important to reduce readmissions."
