"Pharmacists can play a tremendous role in helping with reducing readmissions. Since 2013 many pharmacy departments in acute care hospitals have lead initiatives to directly impact readmissions in targeted disease states. Pharmacist-led interventions include helping assure certain core measures are met in specified disease states at discharge, comprehensive discharge counseling with individual patient tailored education materials, discharge medication reconciliation and post discharge telephone follow-ups.
