  "...our teams combined hundreds of SDOH [social determinants of health] data attributes and created a predictive model to forecast the probability an individual would be readmitted within 30 days. The model performed as well as a model based on clinical/EMR data only and demonstrated that 50 percent of those identified in the highest risk category (~1% of all admissions) were readmitted."
